Income & Expense Data:

● Total number of exemptions claimed is on: IRS Form 1040 – line 6d; 1040A – line 6d; 1040EZ – n/a

● Adjusted gross income is on: IRS Form 1040 – line 37; 1040A – line 21; 1040 EZ – line 4

● Total U.S. income tax is on: IRS Form 1040 – line 63; 1040A – line 39; 1040EZ – line 12

 

Scholarships, grants and other assistance information:

Include any known financial aid or assistance you will receive to help pay for your education. This may include, but is not limited to, scholarships from foundations or organizations; college or university awards (i.e., Presidential Scholarship, Honor Scholarship, athletic, etc.); state grants or scholarships (MEAP Scholarship); and Pell Grants.

 

Household information:

● Total size of the parent(s) household includes the student applying and the parent(s). Include other children if your parent(s) will provide more than half their support from July 1, 2016 through June 30, 2017. Include other people if they currently live with your parent(s) and your parent(s) provide more than half of their support and will continue to provide more than half of their support from July 1, 2016 through June 30, 2017.

 

● Total size of the applicant’s household includes you, your spouse (if applicable), and your children if you will provide more than half of their support from July 1, 2016 through June 30, 2017. Include other people if they currently live with you and you provide more than half of their support and will continue to provide more than half of their support from July 1, 2016 through June 30, 2017.

 

Number in college:

● For the question about the number of people in college, include yourself, even if you are attending college only part-time.

 

● Do not include your parents.

 

● Include others only if they will take more than 6 credit hours per semester in a program leading to a college degree or certificate.

 

Other:

● If your parents are divorced or separated, complete the Financial Assistance Questionnaire based on the information of the parent you lived with more during the past 12 months. If you did not live with one parent more than the other, complete the questionnaire based on the parent who provided more financial support during the past 12 months. If the parent whose information you are reporting has remarried, answer the questions based on your parent and his/her spouse.

 

● If your parents’ (or your) 2016 federal tax return is not finished, you may complete the questionnaire based on a 2015 tax return or you may estimate a 2016 return. In either case, you must submit a copy of the tax return you used to complete the questionnaire and W-2 statements for 2016.  If you do not yet have your 2016 W-2’s, provide us with the 2015 W-2’s until the 2016 W-2’s are available.  As soon as the current year w-2’s are available, send or drop off a copy to us and we will replace them in your application.

 

● You do not need to submit the various schedules that accompany the IRS 1040 form. However, we do need the entire 1040 form, including the signature page (two pages – front and back).
